PAS Manual
LifeStream Services is part of the Indiana's Nursing Facility Pre-admission Screening (PAS) program.
The program is aimed at evaluating an individual's situation and providing information on possible alternatives to admission to a nursing facility. The purpose of PAS is to assure that all alternatives are explored before an individual is placed in a nursing facility.
The program also monitors nursing facility admission to ensure placements are appropriate. Our PAS office is open weekdays during normal business hours to assist with admissions. In addition, qualified personnel from East Central Indiana nursing facilities who need information may access the following document for more details:
