LifeStream Services offers a variety of information and services as well as referrals to other resources to help people with independent living solutions.

Our Information and Referral/LifeStream 2-1-1 office is the place to start. Just call (765) 759-1121 or (800) 589-1121 during regular business hours weekdays. To reach the 2-1-1 Center just dial 2-1-1 if you are in East Central Indiana.

Checklist for Choosing a Nursing Home: This checklist will help when it comes to interviewing personnel and making decisions about nursing home placement. This document was provided by Medicare.

Choosing a Quality Nursing Care Facility: The American Association of Homes and Services for the Aging represents 5,600 not-for-profit nursing homes nationwide and state association partners in 39 states. The organization is among a wide number of resources available that offer suggestions on how to choose a quality facility.

Nursing Facility Profile Reports: The Indiana Department of Health provides a detailed profile of all certified and non-certified comprehensive care nursing facilities within the state of Indiana. To search the profiles, visit http://www.in.gov/isdh/regsvcs/ltc/profile/.

Pre-Admission Screening: Those who must be admitted to nursing facilities in Indiana must go through the pre-admission screening process. LifeStream is the East Central Indiana approved screening site for all placements within the agency's service area. The manual is intended to help with admissions, though professional pre-admission screening (PAS) staff members are available weekdays to help with the process.
